Amy Abrams is a Certified Care Manager, and a member of the National Association of Professional Geriatric Care Managers. She earned a Master of Social Work and Master of Public Health from San Diego State University, and founded Elder Care Guides with her business partner Norman Hannay in 2004.

How do Geriatric Care Managers work with families?

• Assess the needs of the individual and recommend the most appropriate, accessible, and cost-effective services in specific geographic areas, including both in-home options and communal living environments as appropriate
• Monitor and evaluate services as needed, on an ongoing basis
• Coordinate care and support services with sensitivity and respect for the individual’s autonomy and right to self-determination
