Amy Abrams is a Certified Care Manager, and a member of the National Association of Professional Geriatric Care Managers. She earned a Master of Social Work and Master of Public Health from San Diego State University, and founded Elder Care Guides with her business partner Norman Hannay in 2004.

- Assess the needs of the individual and recommend the most appropriate, accessible, and cost-effective services in specific geographic areas, including both in-home options and communal living environments as appropriate
- Monitor and evaluate services as needed, on an ongoing basis
- Coordinate care and support services with sensitivity and respect for the individual’s autonomy and right to self-determination
